29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) At 27331 Gulf Road, Orange Beach, Alabama: on or about July 12, 2017 and at times prior; the employer allowed his employees to work from a high slope roof without fall protection exposing them to a fall hazard of up to 28 feet to the ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) At 27331 Gulf Road, Orange Beach, Alabama: on or about July 12, 2017 and at times prior; the employer allowed his employees to use an extension ladder to gain access to the roof top and the rails of the ladder did not extend at least three feet above the landing area to provide safe access exposing his employees to a fall hazard of up to 18 feet to the deck below.
