29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:      (a) On or about July 13, 2017, and at times prior to that date, for the employees exposed to crushing hazards from the gantry bridge on the Alltra plasma machine, model no. PG14-8, serial no. 5593 while operated in auto mode in the fabrication department at 2130 Third Avenue NW, West Fargo, North Dakota.      (b) On or about July 13, 2017, and at times prior to that date, for the employees exposed to crushing hazards from the gantry bridge on the Alltra plasma machine, model no. PG14-10, serial no. 4876 while operated in auto mode in the fabrication department at 2130 Third Avenue NW, West Fargo, North Dakota.      (c) On or about July 13, 2017, and at times prior to that date, for the employees exposed to crushing hazards from the gantry bridge on the Alltra plasma machine, model no. PG14-12, serial no. 5344 while operated in auto mode in the fabrication department at 2130 Third Avenue NW, West Fargo, North Dakota.    Note: This company was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard which was contained in OSHA Inspection Number 1021091, Citation 1, Item 1, and was affirmed as a final order on 05/01/2015, with respect to a workplace located at 2130 Third Avenue NW, West Fargo, North Dakota 58078.
