29 CFR 1910.22(a)(1): (1) All places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition.    On or about July 13, 2017, and times prior thereto, at Sutton Lumber, located at or near 106 Coker Rd. Amity, Arkansas employees working with a double end trim in the work shop saw routinely, exposed to trip and fall hazards from dust all over the floor.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:      On or about July 13, 2017, and times prior thereto, employees of Sutton Lumber, located at or near 106 Coker Rd. Amity, Arkansas employees are required to work with a double end trim saw and notch machine without specific lockout/tagout procedures for each machine they would have to service and/or repair.  Employees are repeatedly exposed to uncontrolled energy without a lockout/tagout program.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.    On or about July 13, 2017, and times prior thereto, employees of Sutton Lumber, located at or near 106 Coker Rd. Amity, Arkansas were required to clean a trim saw that is not guarded.  Employees are exposed to the hazard of being caught-in or contact with rotating saw blades.

Amity, Arkansas are working with an electric cabinet in the workshop that has a missing knockout. This condition exposed employees to electrical hazards.
