Materials or equipment was placed, either permanently or temporarily, within the exit route:    a) CCT USA LP: On July 14th, 2017 and at times prior to, the employer did not ensure two exit doors located at the North-east corner of the CCT facility were maintained in a free and unobstructed condition. One of the exit doors was locked with a padlock; the other exit door was blocked with company boxes.

Capacity, operation, and maintenance instruction plates, tags, or decals were not changed accordingly.    a) CCT USA LP:  On July 14, 2017 and at times prior to, the employer authorized modifications and additions which affect capacity and safe operations of at least two (2) powered industrial trucks (Nissan Powered industrial truck - model number MCPLO2A25LV, Nissan Powered industrial truck - Model Number MCP1F2A25V) without the manufacturer's prior written approval or a registered engineer's written approval. This condition exposes employees to struck-by and crushing hazards.
