29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.  a.) 181 Clerk St., Jersey City, NJ 07305  On or about 7/13/2017 employees were working on the 2nd floor of a residential dwelling while being exposed to fall hazards approximately 20 ft., due to uncovered floor holes that were approximately 4 ft. X 3 ft., which were created by the removal of a chimney.
