29 CFR 1910.147(c)(2)(ii): An energy isolating device capable of being locked out, the employer's energy control program under paragraph (c)(1) of this section will utilize lockout, unless the employer can demonstrate the utilization of a tagout system will provide full employee protection as set forth in paragraph (c)(3) of this section.  On or about July 12, 2107, employees engaged in maintenance activity on a structural blast machine did not utilize their lockout/tagout devices.  This exposed employees to caught-in hazards.

29 CFR 1910.147(c)(7)(iii)(A): Retraining was not provided for authorized and affected employees when there was a change in their job assignments, a change in machines, equipment or processes that presented a new hazard, or when there was a change in the energy control procedures.  On or about July 12, 2017, employees authorized to perform lockout/tagout procedures were not provided with training prior to performing maintenance activity on a structural blast machine located at a customer's facility. This exposed employees to caught-in hazards.
