The program did not enable each employee to recognize the hazards of falling and did not train each employee in the procedures to be followed in order to minimize these hazards.  a) On or about July 14, 2017 or prior thereto:   The employer did not establish an adequate training program in that fall protection was not utilized for recognized fall hazards on a residential construction site.    Fall hazards ranged from 9 to 24 feet on the site during roof shingle removal, and a monitor system was utilized for a work surface with a pitch greater than 4/12.   The work surface had a roof pitch of 6/12.  NOTE:   Abatement certification is required for this item.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. Exception: When the employer can demonstrate that it is infeasible or creates a greater hazard to use these systems, the employer shall develop and implement a fall protection plan which meets the requirements of paragraph (k) of 1926.502.  a) On or about July 14, 2017 or prior thereto:   On a residential construction site, during roof shake shingle tear off operations, employees were exposed to fall heights ranging from 9 to 24 feet on a roof sloped at 6/12.  Employees were not wearing fall protection.  NOTE:   Abatement certification and documentation are required for this item.  The employer, Julio Alvarado, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number #901780, citation number 1, item number 1 and was affirmed as a final order on May 16, 2013, with respect to a workplace located at  10556 West Palazzo Lane in Boise, Idaho 83707.
