29 CFR 1910.218(f)(2): Hydraulic Forging Presses. When dies were being changed or maintenance was performed, the hydraulic pumps and power apparatus were not locked out and the ram was not blocked. Press Dept., Bliss Hydraulic Forging Press. On or about July 17 through November 16, 2017, employees worked in the die area beneath the ram in order to make adjustments on the die such as tightening bolts. The hydraulic pumps and power apparatus were not locked out. Employees were exposed to crushing hazards. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): Employees were not required to apply energy controls in the following sequence: 1) turn off and shut down the machine, 2) physically locate and operate energy isolating devices in such a manner as to isolate the machines or equipment from the energy source(s), 3) affix lockout or tagout devices to each energy isolating device, 4) verify that isolation and deenergization of the machines or equipment had been accomplished. Press Dept., Bliss Hydraulic Forging Press. Employees were exposed to amputation and crushing hazards when performing the following servicing and maintenance tasks, including setup: a) On or about July 17, 2017, the interlock switches on the fixed barrier cage guard around the perimeter of the press and robot cell were damaged and bypassed and did not prevent unexpected startup when the guard doors were opened. The employer did not ensure that effective lockout/tagout procedures were utilized when adequate alternative protection was not provided. b) On or about July 17 through November 16, 2017, employees entered the fixed barrier cage guard around the perimeter of the press to make adjustments to the die such as tightening bolts. Employees did not deenergize and lockout the press hydraulic pump and did not verify the blocking of the ram. The employer did not ensure that effective lockout/tagout procedures were utilized when adequate alternative protection was not provided. c) On or about July 17 through November 16, 2017, employees entered the robot cell for the purposes of changing grippers (teeth) on the robot, training the robot and taking quality control samples. The employer did not ensure that effective lockout/tagout procedures were utilized when adequate alternative protection was not provided. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to the following: (A) A specific statement of the intended use of the procedure; (B) Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy; (c) Specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them; and (D) Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices and other energy control measures: a) Press Dept., Bliss Hydraulic Forging Press. On or about July 17 through November 16, 2017, employees who performed servicing and maintenance tasks, including setup, were exposed to amputation and crushing hazards when working in the die area beneath the ram to make adjustments such as tightening bolts. Employees were not provided with adequate lockout/tagout procedures that contained a specific statement of the intended use of the procedure for such setup tasks and did not contain a method for deenergizing the hydraulic pump and verifying the blocking of the ram. b) Press Dept., Bliss Hydraulic Forging Press Area, ABB Robot. On or about July 17 through November 16, 2017, employees were exposed to amputation and crushing hazards when entering the robot cell for the purpose of training the robot, changing the grippers (teeth) on the robot and taking quality control samples. Employees were not provided with adequate lockout/tagout procedures that contained a specific statement of the intended use of the procedure for such setup and inspection tasks . Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

On or about July 17, 2017 working space about 480 volt electrical panels was restricted by large chemical totes to less than the 3 foot acceptable clearance indicated in Table S-1, which could expose employees to fire/smoke inhalation and delay in shutting down of equipment in an emergency. No abatement certification or documentation is required for this item.
