29 CFR 1926.252(a): Whenever materials are dropped more than 20 feet to any point lying outside the exterior walls of the building, an enclosed chute of wood, or equivalent material, were not used. For the purpose of this paragraph, an enclosed chute is a slide, closed in on all sides, through which material is moved from a high place to a lower one: a. On July 18, 2017, an employee was tossing roofing material off the roof more than 20 feet above the dumpster that was located near the front door of the house.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section: a. On July 18, 2017, on the 8:12 pitch roof, an employee performing roofing work was not protected from a 22 feet fall hazard by the use of a fall protection system. b. On July 18, 2017, on the 3rd floor balcony, an employee used the unguarded balcony to access the roof, exposing himself to a 20 feet fall hazard.
