29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a)  207 Poplar Street, Richland PA - Employees were observed conducting roofing operations on a multi-pitched residential roof and were not protected from fall hazards up to approximately 20 feet to the ground below, on or about July 17, 2017.  No abatement certification or documentation required.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a)  207 Poplar Street, Richland PA - Employees used a portable Werner MT-22 multi-purpose ladder to access and egress a roof and the ladder extended 31 inches above the landing surface exposing the employees to a fall hazard, on or about July 17, 2017.  No abatement certification or documentation required.

29 CFR 1926.503(a)(1): A training program was not provided to each employee who might be exposed to fall hazards, to enable that employee to recognize the hazards of falling, and be trained in the procedures to be followed to minimize these hazards:  a)  207 Poplar Street, Richland PA - Employees were not trained in appropriate fall protection methods to avoid hazards associated with elevated work areas, on or about July 17, 2017.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
