29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) 624 Randolph St. Front: On or about July 14, 2017 and times prior, employees were exposed to a fall of 12 feet 4 inches eave to ground while installing shingles on a residential roof. Employees were not protected from falling by a guardrail, safety net or personal fall arrest system. b) 624 Randolph St. Left Side: On or about July 14 2017 and times prior, an employee were exposed to a fall of 7 feet 9 inches eave to ground while replacing shingles on a door awning of a residential home. The employee was not protected from falling by a guardrail, safety net or personal fall arrest system.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) Front of home: On or about July 14, 2017 and times prior, employees were exposed to a fall of 12 feet 4 inches while using a 15 foot Werner extension ladder to access the roof to replace shingles. The ladder was not extended 3 feet above the roof edge. b) Left side of home: On or about July 14, 2017 and times prior, employees were exposed to a fall of 7 feet 9 inches while using a 20 foot Werner extension ladder to access the roof to replace shingles. The ladder was not extended 3 feet above the roof edge.
