On or about July 11, 2017, at the above addressed jobsite, an employee was using an unapproved attachment (a wooden job made box) on the forks of a power industrial vehicle/PIV TEREX SNOOK forklift to hoist hot water heaters to a third story balcony, thereby exposing employees to fall and struck by hazard.
