29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee on a scaffold more than 10 feet (3.1 m) above a lower level was not protected from falling to that lower level by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:  On or about July 20, 2017, at a work site in the Sam's Club located on Ambassador Caffery Parkway in Lafayette, LA, the employer did not ensure that an employee using a JLG scissor lift to install the drain line for evaporator coils in the new produce cooler was protected from falling approximately 18' to the floor below.  The mid-rail chain on the scissor lift was not closed to complete the guardrail system.
