29 CFR 1926.252(a): An enclosed chute of wood, or equivalent material, was not used where materials were dropped more than 20 feet to points lying outside the exterior walls of the building(s): (a) Broadstone Windy Ridge, East side of building: On or about 07/20/2017, and at times prior, the employer exposed employees located near the 30-yd containers to overhead struck-by hazards, in that trash chutes or other safe means to discard sheetrock debris from heights of approximately 64 feet was not provided.

29 CFR 1926.503(a)(2): The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii). a) Broadstone Windy Ridge, East side 5th floor apartment: On or about 07/20/2017, and at times prior, the employer failed to provide fall protection training to employees working from a balcony at heights of 64-feet, in that an essential anchor for the personal fall protection system was not being used by employees removing debris from the balcony.
