29 CFR 1910.22(a)(1): Places of employment, passageways, storerooms, service rooms, and walking-working surfaces were not kept in a clean, orderly, and sanitary condition: On or about July 21, 2017, employees accessing the formulations laboratory were exposed to trip hazards in the hallway. Lines pumping solvents to the lab and waste from the lab were laid on the floor throughout the hallway.

29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer: On or about July 21, 2017, the employer failed to provide basic respirator information to employees who wear respirators on a voluntary basis while dispensing solvents including, but not limited to, methylene chloride, ethyl acetate, DMSO, and benzyl alcohol.

29 CFR 1910.157(g)(2): The educational program to familiarize employees with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting was not provided to all employees upon initial employment, and at least annually thereafter: On or about July 21, 2017, employees were expected to utilize portable fire extinguishers throughout the facility to put out incipient stage fires. The employer failed to provide training to the designated employees regarding the use of fire extinguishers and the hazards associated with incipient stage firefighting upon initial hire and annually thereafter. The company last provided this training on June 22, 2015.
