29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  (a) Top of roof on north side of house - On or about July 20, 2017 employees framing the roof on the third floor were not protected from falling with the use of a fall protection system.

29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided:  (a) Top floor roof access on north side - On or about July 20, 2017 two employees framing the third floor roof gained access to the roof by climbing the outside of the building 7 feet high. No ladder was provided.
