29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a)  891 Campbell Boulevard, Amherst, NY - On or about 7/11/17, employees framing and sheathing a2 story house were cutting and nailing down sheathing on a roof with an eave height of approximately 19 feet.  There were no fall protection measures in place.  NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.501(b)(15): Each employee on a walking/working surface 6 feet (1.8 meters) or more above lower levels was not protected from falling by a guardrail system, safety net system, or personal fall arrest system:  a) 891 Campbell Boulevard, Amherst, NY - On or about 7/11/17, employees were framing, cutting sheathing, and sheathing the front roof area of a new 2 story house.  One employee was using a sheet of plywood for a platform supported by the forks of a JCB Loadall all-terrain lift approximately 19'.  There were no fall protection measures in place.  b) 891 Campbell Boulevard, Amherst, NY - On or about 7/11/17, employees were framing, and sheathing the front roof area of a new 2 story house.  One employee was using a sheet of plywood for a platform supported by the forks of a JCB Loadall all-terrain lift approximately 12'.  There were no fall protection measures in place.   NO ABATEMENT CERTIFICATION REQUIRED
