29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) On or about 07/24/17, on the main roof and front dormer roof on a house at 2170 Main St., Buffalo, NY: Two employees engaged in roofing work on a steep roof were exposed to a fall of approx. 9 to 21 foot to the porch roof or the ground below. No fall protection system was being utilized. Pitch of the main roof was approx. 8 in 12. NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) On or about 07/24/17, at the front of the house at 2170 Main St., Buffalo, NY: Employees used an extension ladder to access the porch roof which was approx. 12 ft. above the ground. The side rails of that ladder only extended approx. 20 inches above the porch roof. NO ABATEMENT CERTIFICATION REQUIRED
