29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a. Checker Notions, LLC - Maumee, Ohio: On or about July 25, 2017, the employer did not ensure that Carousel #1 was adequately guarded to protect employees from pinch point/crushing injuries. Employees were exposed to crushing injury where pod 1 passes the fixed conveyor on the right and where carriers/bins close on the left facing side of pod 2. b. Checker Notions, LLC - Maumee, Ohio:On or about July 25, 2017, the employer did not ensure that Carousel #2 was adequately guarded to protect employees from pinch point/crushing injuries. Employees were exposed to crushing injury where pod 2 passes the fixed conveyor and where carriers/bins close on the right facing side of pod 1. c. Checker Notions, LLC - Maumee, Ohio:On or about July 25, 2017, employees operating Carousel #3 were exposed to crushing injuries from moving components of the pods in that the equipment was not adequately guarded to protect employees. Employees were exposed to pinch points created as the pods rotated past equipment such as, but not limited to, stationary fencing and steps. d. Checker Notions, LLC - Maumee, Ohio: On or about July 25, 2017, employees operating Carousel #4 were exposed to crushing injuries from moving components of the pods in that the equipment was not adequately guarded to protect employees. Employees were exposed to pinch points created between bins and as the pods rotated past equipment such as, but not limited to, stationary fencing and steps. e. Checker Notions, LLC - Maumee, Ohio: On or about July 25, 2017, employees operating Carousel #5 were exposed to crushing injuries from moving components of the pods in that the equipment was not adequately guarded to protect employees. Employees were exposed to pinch points created between bins and as the pods rotated past equipment such as, but not limited to, stationary fencing and steps. f. Checker Notions, LLC - Maumee, Ohio: On or about July 25, 2017, employees operating Carousel #6 were exposed to crushing injuries from moving components of the pods in that the equipment was not adequately guarded to protect employees. Checker Notions, LLC - Maumee, Ohio: On or about July 25, 2017, employees operating Carousel #9 were exposed to crushing injuries from moving components of the pods in that the equipment was not adequately guarded to protect employees. Employees were exposed to pinch points created between bins and as the pods rotated past equipment such as, but not limited to, stationary fencing and steps. j. Checker Notions, LLC - Maumee, Ohio: On or about July 25, 2017, employees operating Carousel #10 were exposed to crushing injuries from moving components of the pods in that the equipment was not adequately guarded to protect employees. Employees were exposed to pinch points created between bins and as the pods rotated past equipment such as, but not limited to, stationary fencing and steps.
