29 CFR 1910.25(b)(3): The employer did not ensure that the stairs had uniform riser heights and tread depths between the landings:  A.  On or about July 26, 2017, in the following locations, riser heights were not uniform between the landings:      1.  Lower parking lot, left staircase:  From the bottom, the first step was 12 inches from the ground, the next three steps had 7 1/2-inch risers, and the landing was 10 1/2-inches above the top step.      2.  Lower parking lot, right staircase, Prospect Hill Road side:  From the bottom, the first step was 3-inches from the ground, the next four steps had 7 1/2" risers, and the landing was 10 1/2-inches above the top step and over a 2 3/4-inch curb.

29 CFR 1910.28(b)(11)(ii): Each flight of stairs having at least 3 treads and at least 4 risers was not equipped with stair rail systems and handrails in accordance with Table D-2, Stairway Handrail Requirements:  A.  On or about July 26, 2017, in the following locations, flights of stairs were not equipped with stair rail systems and handrails:  1.  Lower parking lot, right side, near Prospect Hill Road:  A 46-inch wide stair system having 5 risers lacked stair rail protection on the left open side, ascending.  2.  Lower parking lot, left side:  A 48-inch wide stair system having 5 risers lacked stair rail protection on the left and right open sides.
