29 CFR 1910.23(c)(4): (4) Ladders are used only on stable and level surfaces unless they are secured or stabilized to prevent accidental displacement;    a) Jobsite: A ladder was placed on the top of the working level of a rolling scaffold to reach and paint a window that was at a higher level. The ladder was displaced and the scaffold tipped over causing employees to fall.      Condition was noted on or about 19 JUL 2017.
