29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):    The employer allows workers to be exposed to hazardous conditions created by not conducting frequent or regular inspections.  The most recent occurrence was near Caddy Ct., St. Clair, MO, where they failed to make frequent or regular inspections of the worksite to ensure all workers were in compliance with OSHA standards.   The workers were exposed to a fall hazard ranging from approximately 10 feet to 18 feet as they were performing residential roofing activities on an approximate 6:12 pitch without the use of any fall protection system.  In addition, employees failed to wear fall protection while working from aerial lifts.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:     The employer is failing to protect employees from fall hazards.  This condition was observed at the newly constructed residence located near Caddy Ct., St. Clair, MO  63077 when workers were exposed to a fall hazard ranging from approximately 10 feet to 18 feet as they were performing residential roof sheathing on an approximate 6:12 pitch without the use of any fall protection system.
