29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.     On or about July 26, 2017, employees performing residential roofing installation were exposed to a 11.1 foot fall when the employer failed to ensure that fall protection was utilized.     No abatement certification or documentation is required of this item.

29 CFR 1926.451(g)(1)(iii): Each employee on a crawling board (chicken ladder) shall be protected by a personal fall arrest system, a guardrail system (with minimum 200 pound toprail capacity), or by a three-fourth inch (1.9 cm) diameter grabline or equivalent handhold securely fastened beside each crawling board.  On or about July 26, 2017, employees utilizing a crawling board (chicken ladder) while performing residential roofing installation were exposed to a 11.1 foot fall when the employer failed to ensure that fall protection was utilized.   No abatement certification or documentation is required for this item.
