29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. On or about July 19, 2017, at 1707 Metro Drive, Alexandria, LA the employer failed to ensure that employees were protected from a fall hazard while conducting roofing activities on a low slope roof.

On or about July 19, 2017, at 1707 Metro Drive, Alexandria, LA the employer did not ensure that ladders used for access and egress to/from upper levels of the structure were used for the purpose for which they were designed. Instance 1) Employee(s) used a 10 foot A-frame ladder to gain access to the awning of the structure that was eleven feet in height. Instance 2) Employee(s) used a 6 foot A-frame ladder that was in the closed position with the feet resting on the awning of the building and the top secured to the roof.
