29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system: a) On or about 07/26/17, on a jobsite at a house on 21 Wildwood Pl., Buffalo, NY: Two employees were exposed to a potential fall of approx. 15 ft. from the pik on a Ladder Jack Scaffold while doing carpentry work at the roof overhang. No Fall protection system was being utilized. NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: a) On or about 07/26/17, on a jobsite at a house on 21 Wildwood Pl., Buffalo, NY: An employee working on the ground doing clean-up work in the vicinity below employees working above from a Ladder Jack Scaffold was exposed to the potential hazard of being struck by materials/tools that could fall off the scaffold. The employee was not wearing a hardhat. NO ABATEMENT CERTIFICATION REQUIRED
