29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a)  Web Room, TRC Auto-Cutter Machine (TRC 1000 Strip Cutter).  On or about July 11, 2017 through July 31, 2017 employees were exposed to amputation, laceration and crush hazards to fingers due to missing or inadequate guards on the rollers and heated guillotine knife blade area of the machine.     b)  Tuflex Dept., TRC Auto-Cutter Machine.  On or about July 31, 2017, employees were exposed to amputation, laceration and crush hazards to the fingers due to inadequate guards on the roller and rotary cutting blade areas of the machine.     Abatement documentation is required for this item in accordance with the requirements of 1903.19(d).

29 CFR 1910.212(a)(2): Guard(s) on machine(s) were not affixed to the machine or secured elsewhere when attachment to the machine was not possible:  Tuflex Dept., TRC Auto-Cutter Machine.  On or about July 31, 2017, the expanded metal guard for the rotary blade for the cutter had two openings in it with moveable doors that were not affixed, exposing employees to amputation and laceration hazards.  There is no abatement documentation or certification required for this item.

29 CFR 1910.147(c)(6)(ii): The employer did not certify that periodic inspections of the energy control procedures had been performed:  Entire Facility.  On or about July 31, 2017, the employer did not certify that a periodic inspection of lockout/tagout procedures had been conducted at least annually for machines and equipment such as exhaust fans, heating units, compressors, welders and other plant machinery, including a review with each authorized employee of their responsibilities under the energy control procedure(s) being inspected.  There is no abatement documentation or certification required for this item.
