29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a) 1290 New York Avenue, Huntington Station, NY : Employees engaged in septic tank installation inside an excavation were working underneath bucket operations of a komatsu Excavator truck (Model #PC 200LC-7L / Serial # A87554) without protective helmets ; on or about 7/28/17.   Note: Because abatement of this violation was already document in the case file, the employer need not to provide abatement certification or documentation for this item in accordance with 29 CFR 1903.19.
