29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):  a. On or about July 31, 2017, on the south side of a 6:12 pitch roof, three employees performing roofing operations were not protected from a 10-foot 7-inch fall hazard by use of a guardrail system, safety net system, personal fall arrest system or any alternative fall protection.   b. On or about July 31, 2017, on the north side of a 6:12 pitch roof, an employee performing roofing operations was not protected from a 11-foot 7-inch fall hazard by use of a guardrail system, safety net system, personal fall arrest system or any alternative fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a. On or about July 31, 2017, four employees used a 24-foot Werner extension ladder to access a roof improperly, in that the ladder was not extended 3 feet over the eave nor was it secured from slippage, exposing employees to a fall hazard.
