29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used:  a) Worksite 5 Atlantic Ave. East Rockaway, NY- Employee cutting plywood sheathing for exterior framing used a circular saw plugged in with a damaged electrical cord; on or about 7/28/17.  Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) Worksite 5 Atlantic Ave. East Rockaway, NY- Employee installing sheathing to exterior framing of a multi-unit residential construction was exposed to falls of approximately 12 feet; on or about 7/28/17.  Note:  In addition to abatement certification the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.
