The ground prong provided on the male plug for the power tap was hanging over the edge of the female receptacle on the extension cord.

29 CFR 1910.334(a)(3)(iii): Adapters which interrupt the continuity of the equipment grounding connection were used:   Couch Area:  On, or about, August 1, 2017, an overhead light fixture was plugged into a receptacle through a cheater plug that was designed to allow a three wire male plug to be inserted into a two wire female outlet.  Because the grounding conductor on the cheater plug was not attached the cover screw on the female outlet, the path to ground was not continuous.
