29 CFR 1926.20(b)(1): The employer did not initiate and maintain accident prevention programs as necessary to comply with this part:  (a) Town & Country Log Home LLC on or about July 21, 2017, at the address of 22100 Lake Rd. Rocky River, OH: The employer did not have a plan or program to identify job site hazards which assists the employer and employees in maintaining a safe working environment.  Such programs would support the employer and employees in identifying and avoiding potential hazards commonly found during employees work activities, decreasing the probability of employee injury and/or illness.

29 CFR 1926.304(i)(1): Each circular had-fed ripsaw was not guarded by a hood which completely enclosed that portion of the saw above the table and that portion of the saw above the material being cut.  a) Town & Country Log Home LLC on or about July 21, 2017, at the address of 22100 Lake Rd. Rocky River, OH:  The employer did not ensure a Ryobi 10 inch table saw (model #RTS216) was equipped with the appropriate factory provided guard while in use.  An employee using the unguarded tool to cut wooden window framing material was exposed to potential contact of the blade during cutting operations.
