29 CFR 1926.502(i)(4): All covers were not color coded or marked with the word "HOLE" or "COVER" to provide warning of the hazard:    (a) 1401 Spruce St., Philadelphia, PA 19102, 16th Floor - The employer did not ensure that covers installed over floor holes, including small holes and a large hole through which a person could fall that was at a height of approximately 12 feet above the lower level, were marked with the word "HOLE" or "COVER", thus exposing employees to tripping and fall from height hazards.
