29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) while working from an aerial lift:      a) 61 Hwy. 1 South, Metuchen, NJ 08840. (Tilted Kilt Restaurant) Employees were working on and from the basket of a Dur-A-Lift telescopic aerial lift that was mounted on a 2006 Ford E350 truck with no fall protection system in place exposing them to fall hazards in excess of 13 feet, 8 inches from the basket to the ground below.       Occurred on 08/01/2017.

29 CFR 1926.501(b)(1): Employee on a walking/working surface having an unprotected side or edge which is six feet or more above a lower level were not protected from falling by the use of a guardrail system, a safety net system, or personal fall arrest system:      a) 61 Hwy. 1 South, Metuchen, NJ 08840. (Tilted Kilt Restaurant) Employees were exposed to fall hazards in excess of 13 foot, 8 inches from the eave to the ground below while installing a new sign on the roof with no fall protection systems in place.      Occurred on 08/01/2017.
