29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided.  Site: 30 Bond Street New York, NY  On or about 8/2/17  a) Employees were accessing their work area from the 7th floor window and from the window frame to the scaffold. There was a break in elevation greater thn 19 inches as measured by CSHO.
