29 CFR 1910.178(l)(1)(ii): The employer did not ensure that each operator had successfully completed the training required by paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate a Power Industrial Truck: On or about August 4, 2017, the employer did not ensure that employees received a combination of formal instruction, practical training and an evaluation of the operator's performance in the workplace prior to permitting each employee to operate a powered industrial truck, exposing employees to the hazards associated with operating a Powered Industrial Truck. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer: On or about August 4, 2017, an employee was voluntarily using a 3M N95 dust mask, the employee was not provided with appendix D or training on the proper use and limitations of the respirator, exposing the employee to an inhalation hazard. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
