29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system:      Instances including, but not limited to, the following:    a) Job site, 124-134 Muirfield Drive, West View, PA: On or about August 1, 2017, during roof sheathing and truss framing activities, the employer did not ensure that appropriate fall protection systems/devices were used where employees were exposed to fall hazards of up to approximately 28 feet.     b) Job site, 124-134 Muirfield Drive, West View, PA: On or about August 1, 2017, during construction activities, the employer did not ensure that appropriate fall protection systems/devices were used where wall openings existed in that a wall opening for a window, was determined to have a lower sill approximately 2 feet from the floor and an open area of approximately 3 feet long by 5 feet tall, with a fall hazard of approximately 12 feet, 8 inches.    c) Job site, 124-134 Muirfield Drive, West View, PA: On or about August 1, 2017, during construction activities, the employer did not ensure that appropriate railings were provided on a stairway from the first floor to the second floor.    d) Job site, 124-134 Muirfield Drive, West View, PA: On or about August 1, 2017, during construction activities, the employer did not ensure that employees were properly protected from falling or stepping into holes in that a hole on the roof did not have a set of guardrails around it or was not provided with a cover.
