29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems or a combination of warning line system and safety monitoring system.   (a)  On or about July 24, 2017, and at times prior thereto, the employer did not ensure a proper fall protection system was implemented prior to performing roofing activities and exposing employees to a 12 foot 5 inch fall hazard.  Note:  Abatement certification IS required for this item.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section (fall protection training) by preparing a written certification record which contained the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer:  (a)    On July 24, 2017, and at times prior thereto, The employer did not prepare a written certification verifying that employees had been trained in hazards related to falls from elevation. 29 CFR 1926.503(c): When the employer has reason to believe that any affected employee who has already been trained does not have the understanding and skill required by paragraph (a) of this section, the employer is to retrain each such employee such as inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding of skill.  (a)  On or about July 24, 2017, and at times prior thereto, Employees working without fall protection demonstrated inadequacies in their knowledge to effectively recognize, identify and/or implement the procedures to be followed in order to minimize potential fall hazards.  Note: Abatement certification IS required for this item.
