29 CFR 1915.115(d): Accessible areas within the swing radius of the outermost part of the body of a revolving derrick or crane, whether permanently or temporarily mounted, shall be guarded in such a manner as to prevent an employee from being in such a position as to be struck by the crane or caught between the crane and fixed parts of the vessel or of the crane itself. a) On or about 08-08-2017 and times prior thereto, located at 3400 Shipwright St. Portsmouth, VA 23703, Condition: Employee(s) were exposed to struck-by and caught-between hazards while line handling and signaling within the swing radius near the base of the unguarded permanently mounted revolving derrick. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.

29 CFR 1915.116(q): An employee was permitted to place him/herself in a hazardous position between a swinging load and a fixed object. a) On or about 08-08-2017 and times prior thereto, located at 3400 Shipwright St. Portsmouth, VA 23703, Condition: Employee(s) were exposed to struck-by and leg amputation hazards from working in a hazardous area between a suspend and swinging load and a fixed concrete derrick base. The employee, while trying to push and maneuver the suspended vessel by hand, was struck-by and caught-between the vessel and derrick concrete base when the load shifted resulting in a right leg amputation above the knee. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.
