29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  b - 5 Connell Court, Block 701 - Lot 3.32, Mount Laurel NJ: Company employees (not using fall protection) were exposed to fall hazards, in excess of 9 feet to the ground surface below, when accessing and egressing a residential building's second story using the fly section of an aluminum extension ladder that did not extend at least three feet above the upper landing surface being accessed.
