29 CFR 1926.403(b)(1): The employer shall ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees. Safety of equipment shall be determined on the basis of the following considerations:  Classification by type, size, voltage, current capacity, specific use.  At north end of the building the yellowish/orange AWG 16/3 extension cord was providing power a hand held circular saw. The cord was curled - presenting signs of having been overloaded. Employees were exposed to the hazard of contacting a damaged cord which may present a burn and/or shock hazard.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws :  On the north end of the building the employer did not ensure that the male plug of the AWG 16/3 yellowish orange flexible cord was properly attached to the cord. The employee was exposed to a shock hazard.

29 CFR 1926.501(b)(13): Employees engaged in residential construction activities 6 feet (1.8m) or more above the lower levels were not protected by guardrail systems, safety net systems, or personal fall arrest systems:  The employer did not ensure that the employee performing siding installation on the roof of the residence was protected from falling. The employee was exposed to a fall hazard of approximately eleven (11) feet six (6) inches from the eave to the ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  The employer did not ensure that the ladder being utilized to access the roof where siding was being installed was extended at least three feet above the landing surface of the roof. It was extended approximately 18 inches. The employee was exposed to a fall hazard of approximately eleven (11) feet six (6) inches from the eave to the ground below.
