Employees were utilizing pneumatic air guns without out the use of safety rated glasses.

Two employees were observed working from an aerial lift without the use of safety harnesses or lanyards.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about August 8, 2017 the employer failed to ensure that ladders used to access the roof at the job site were extended at least 3 feet above the roof edge. The ladder was extended approximately 12 inches above the landing when employees were observed descending from the roof.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.   a)  Charm Builders, Ltd. on or about August 8, 2017, at the address of 47 2nd Street NE, Massillon OH: The employer failed to ensure employees were protected from falls while installing material from a steep sloped commercial type structure. Employees working in this condition were exposed to falls of approximately 30-40 feet to the ground below.  Charm Builders, Ltd. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(11), which was contained in OSHA inspection number 783421, citation number 1, item number 1 and was affirmed as a final order on August 14, 2013, with respect to a workplace located at 205 West 4th St. Port Clinton, OH.
