29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c).  Worksite at 502 E 6th Ave. Stillwater, OK 74074. On or about August 09, 2017, no protective devices were in place to protect employees while working in a trench at a depth greater than five (5) feet.  Employees were exposed to the hazards of being crushed by falling soil.
