29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) Location - 253 Appleton Street, Lowell, MA (Left Side):  On or about 8/10/2017, the ladder used to gain access to roof did not extend at least three feet above the edge of the roof despite being long enough to do so.  29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:  (b) Location - 253 Appleton Street, Lowell, MA (Right Side):  On or about 8/10/2017, the ladder used to gain access to the roof was not long enough to extend three feet past the edge of the roof, and was not secured at the top and provided with a grasping device.
