29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     Building 400:  Employees were engaged in roofing activities without the use of fall protection exposing them to a fall of approximately forty (40) feet to the ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    Building 400 Entrance near mail boxes:  Employees were using an aluminum extension ladder to access the entrance way overhang.  The ladder did not extend three (3) feet above the landing and was not secured.  This exposed employees to falls of approximately ten (10) feet to the concrete and ground below.

29 CFR 1926.1053(b)(6): Ladders were used on surfaces which were not stable and level:    Building 400 entrance near mail boxes:  Employees were using an aluminum step ladder on a sloped roof that was not secured to prevent displacement exposing employees to a fall of approximately ten (10) feet to the concrete and ground below.
