29 CFR 1910.1001(j)(5)(i): The employer did not affix signs or labels to materials that were previously identified to be asbestos-containing material (ACM) and/or presumed asbestos-containing material (PACM) so that employees would be notified of what materials contain ACM and/or PACM.       a) On August 9, 2017, Northridge Preparatory School, Inc. did not ensure that labels or signs were affixed to previously identified asbestos-containing materials or presumed asbestos-containing materials located in the basement, first and second floors of the building.

29 CFR 1910.1001(j)(7)(iv): The employer did not provide to all employees who perform housekeeping operations in areas where ACM and/or PACM is present, an asbestos awareness training course which contained the following elements: the health effects of asbestos, locations of ACM, recognition of ACM and PACM damage and deterioration, requirements in this standard related to housekeeping, and/or proper response to fiber release episodes, at least once a year.      a) On August 9, 2017, Northridge Preparatory School, Inc. did not provide maintenance employees who work in areas where asbestos-containing materials and/or presumed asbestos-containing materials are present with  asbestos awareness training.

29 CFR 1926.1101(k)(2)(ii)(C): The building owner and/or facility owner did not notify all employers of employees who work within or adjacent to the areas containing asbestos containing materials and presumed asbestos containing materials of the presence, location and quantity of such material.      a) On or about August 1, 2017, Northridge Preparatory School, Inc. did not inform all employers, including the heating and air conditioning, plumbing, electric and cleaning contractors, of the presence, location and quantity of asbestos-containing material and/or presumed asbestos containing material present at the site.
