29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about 08/14/2017, at 3465 East Lakeshore, Baton Rouge, LA, employees were conducting roofing work on a residential home at a height of approximately 12 feet without a means of fall protection.

29 CFR 1926.1053(b)(1): Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top to a rigid support that would not deflect and a grab rail provided to assist the employee in mounting and dismounting the ladder:  On or about 08/14/2017, at 3465 East Lakeshore, Baton Rouge, LA, employees were conducting roofing work from a residential home and were using ladders to access the roof that were not 3 feet above the eave of the roof and secured.
