29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measures under another provision of paragraph 1926.501 (b):   JOB SITE: 725 Mescalero Court, Florissant, MO 63031, on or about 8/14/2017, workers were observed on top of a multiple story apartment complex, actively repairing clay roof tiles, approximately 23 ' 1" eave to ground level, and were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measures.  The workers were exposed to a fall hazard.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  JOB SITE: 705 & 725 Mescalero Court, Florissant, MO 63031, on or about 8/14/2017, workers were observed using a Louisville Fiberglass extension ladder, Model: FE-3232 and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  The workers were exposed to a fall hazard.
