On or about August 10, 2017 at 175-17 Liberty Avenue, Jamaica, NY 11433 (Warehouse) The employer failed to ensure a catalytic converter shearing machine used by employees was properly guarded at the point of operation exposing employees to amputation hazards. ABATEMENT CERTIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19

29 CFR 1910.1200(e)(1): Employers shall develop, implement, and maintain at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, safety data sheets, and employee information and training will be met, and which also includes the following: A.) On or about August 10, 2017 at 175-17 Liberty Avenue, Jamaica, NY 11433 The employer failed to develop, implement and maintain at the workplace a written hazard communication program for employees that are required to use hazardous chemicals such as but not limited, propane, during the changing of forklift propane tanks. ABATEMENT CERTIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19
