29 CFR 1926.405(g)(1)(i): Flexible cords and cables were not suitable for conditions of use and location: On or about August 14, 2017, employees were working on the construction of a residential home. They were using multiple extension cords that were plugged into a 3 way plug that was itself plugged into another extension cord. Employees were exposed to electrical shock and fire hazards.

Two employees were observed working without fall protection. Employees were exposed to fall hazards of more than six feet to the ground below.
