A stairway, ladder, ramp or other safe means of egress shall be located in trench excavations that are 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees. There was no ladder on-site.

(a)    Off Lakeland Drive behind the Dollar General in Flowood - On or about May 11, 2017 an employee was working inside at least 5.8 feet deep trench with spoil pile directly next to the excavation. The spoil pile was not moved 2 feet away from the edge of the excavation.
